Copyrighted Work Identification
Clearly identify the copyrighted work you believe has been infringed.
Provide a URL or description of where the original copyrighted work can be found.
If your work is registered with the U.S. Copyright Office, include the registration number.
The year the copyrighted work was created or first published.
Infringing Content Identification
Provide the exact, complete URLs where the infringing content can be found. Be as specific as possible.
Describe how the material infringes your copyright and how it relates to your original work.
Provide enough information for the service provider to locate and identify the infringing content.
Required Legal Statements
Statement that you have a good faith belief that the use is not authorized by the copyright owner, its agent, or the law.
Statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act.
Confirm you have considered whether the use constitutes fair use before submitting the notice.
Signature
Your physical or electronic signature. Typing your full name constitutes an electronic signature.

Capture screenshots of the infringing content as evidence before submitting the notice.
Review the specific requirements of the platform you are submitting to, as they may have additional requirements.
Keep a copy of your notice and any confirmation numbers or case IDs for your records.
Schedule a follow-up date to check on the status of your notice if no response is received.
